Incident Report Template. Record date time location participant staff involved witnesses and incident type. Describe what happened using factual sequence only. Record immediate response, first aid, emergency services, contacts made and supervisor notification. Add current risk level, actions to prevent recurrence, referrals required and whether reportable incident notification may be required. Manager review should record outcome and improvement actions.
